NEW STUDENT ENROLLMENT INFORMATION

New students that reside in the Meade School District are required to register at each school.  The following forms are needed for registration:

 

• Enrollment Form

• Health Services Forms

• Request for Release of Information

• Computer/Technology Forms

     MS/HS Technology Form
     Elementary Technology Form

New students that do not live in the Meade School District, will need to fill out the open enrollment form below for approval/disapproval:

All new students are requested to bring a report card or transcript of grades, and immunization records from the previous school attended. Students will not be admitted to school until and unless the school nurse has received and verified the immunization documentation. 

 

New students must also bring a certified copy of their birth certificate or affidavit in lieu of the birth certificate as issued by the Department of Health if the original birth certificate is deemed unattainable. They will also be asked to provide race and ethnicity information. Meade School District will request the official school records from the former school.
