Monument Clinic will be offering free athletic physicals year round if scheduled with a physician assistant and/or nurse practitioner by appointment only.  Please call 720-2600 to make this appointment.

VERY IMPORTANT:  Urgent Care CAN NOT be used for this physical, an appointment must be made.

Athletic physical forms and the pre-participation history forms (with parental signature) must be completed prior to the athletic physical being performed.
